【摘 要】目的：主要分析慢性病健康管理模式在社区家居养老服务中的应用。方法：于2018年1月构建社区慢性病健康管理小组，发挥小组先进性，对社区慢性病患者进行健康管理服务，评价实施前后社区居家养老服务水平变化。结果：比较实施前后社区慢性病管理的健康体检评分、健康教育评分、成员满意度评分等，实施后优于实施前，P<0.05。结论：在社区居家养老服务中，推广慢性病健康管理模式具有先进性，值得推广。
